Visa/Work Permit Application - Step 2
To All Applicants for a Visa or Work Permit
Please read this page before proceeding to the next step. The application questionnaire that follows is a ‘universal’ questionnaire that will obtain all of the information required for work permits and visas between most countries. If additional information is required for the particular country to which you are going we will let you know once we see your responses. The same questionnaire is used for all trips, whether they be for one day or multiple years. The requirements and processes required for a work visa or permit are the same regardless of the duration of the work.
Please complete all sections with as much detail as possible. All sections must be completed for us to prepare a petition for you. Once we have received the completed questionnaire, we will review it to determine the proper visa/work permit(s) to apply for, prepare the paperwork for you, and send it to you with all the required petition documentation (except your own personal documents) and full instructions on how to proceed. If we require any further information for your particular petition we will contact you at the e-mail address you provided.
If your passport is not up-to-date, please begin the process to renew it. We do not recommend that anyone travel internationally or cross border without a current passport. Also begin gathering any documents that may be required by the immigration officers such as: proof of your education (original diplomas are preferred - including transcripts), proof of your citizenship (citizenship card or passport), permanent resident card if applicable, and proof of your marriage if crossing with a spouse. There should be no other documents required, however, it is always a good idea to take copies of recent pay stubs or a couple of recent bank statements and some utility bills that prove your residence in Canada. A recent employment letter (such as you might use when applying for a loan or mortgage) is also of value. If when we review your questionnaire we see that your circumstances will require additional documentation we will advise you.
While Canada has not started verifying your trips into and out of the country, other governments have, so it is a good habit to begin saving all of your boarding passes and itineraries for future trips. Note that you should keep proof for both business and pleasure trips.
Note: Information entered on this questionnaire may be shared with your company’s HR Department or audit team if requested. If you are on certain IBM assignment plans, some of this information may also need to be shared with IBM's accounting firm for tax purposes.
