Visa/Work Permit Application - Step 1

Note: Use the following questionnaire for visas and work permits only if you are applying to come to Canada.

Do not use this form for any requests for visas or visa renewals to the USA.  IBM Corporation (IBM US) is now processing all visas into the USA (from anywhere in the world).  Contact usvisas@us.ibm.com for information on how to apply for a US visa.

Please ensure that you meet the following requirements before attempting to request a Visa or Work Permit: 

• You must have a copy of your current resume (CV) to be uploaded with the form (maximum file size: 2MB);
• You must have a current passport (valid for duration of trip/assignment plus six months) - we will need you to enter details from that document;
• You must know your IBM Employee number;
• You must know your manager's name and telephone number;
• You must have a confimed job in the foreign country;
• You must know the exact date that you intend to enter the foreign country.
• You MUST have a valid credit card to process the request (unless you are on an approved IBM I/A, STFS, GMF, or HCCM program).

NOTE:  Please allow 15 - 20 minutes to complete the questionnaire - you will not be able to save a partially completed questionnaire for editing or submission at a later time.
